Laparoscopy-assisted Major Liver Resection




Abstract
Laparoscopic surgery is being continuously developed and its indications have been widened to variable organs and disease. Recently, laparoscopic liver resection has been reported in a few series. We experienced a major liver resection that was
performed with laparoscopic assistance. The case was a 44 years old man who had DM and cholecystectomy history 30 years ago. The patient was diagnosed as the multiple impacted intrahepatic duct stones and common bile duct stones. Preoperative
laboratory
findings, including the liver function test, were normal. We planed the left lobectomy of liver due to multiple impacted stones at medical and lateral segment of the left liver. First, 10§® trocar inserted at right upper subcostal and
infraumbilical
area for instruments and camera. Resection of liver was also performed through separated mini laparotomy incision(10§¯) at right subcostal area. The used instruments were electro-cautery, Ultrasonics and Ligasure, etc. The patient's state was
very
stable during intraoperative and postoperative period and discharged at postoperative 18th day without any complication and sequale. Laparscopy-assisted major liver resection could be applied in major liver resection. However, we should be
cautions in
deciding the indication of this procedure, until it proves to be safe.
